Analyze Data

  • Use multiple data sources and geo-mapping, include our proprietary NEST tool and Health Disparity Dashboard
  • Identify racial and ethnic disparities among Medicaid populations, counties, Providers.

Design Initiatives

  • Activate community stakeholders, and engage in provider and SDoH partnerships
  • Target members, providers, and communities for holistic intervention

Factors of Health Inequity

1.    Vulnerable Populations 

Racial and ethnic minorities, low-income individuals, LGBTQIA+ communities, immigrants, people with disabilities, mental health concerns, and seriously mentally ill (SMI).

2.    Social Determinants of Health

Access to quality healthcare, education, employment, housing, and environmental conditions, significantly impact health outcomes and contribute to disparities.

3.     Intersectionality

Intersectionality plays a critical role as individuals may face multiple forms of discrimination or disadvantages that compound health inequities.
